Microsoft Access Project #2

This project will build on the database you created in Project 1.  You will be adding a form for each of your tables and two queries to pull data from the tables.

Your Database

I would suggest you make a copy of your Project 1 database in case you need to start over on Project 2 at some point.  Just copy the database and then rename the copy in case you need it.  The original can then be used for Project 2, but you would have the copy to fall back on if needed.

Forms

You will need to create a form for each of your tables.  This is a simple process in Access 2007, but you can look at the tutorials before doing it if you want to make sure it works (see links).

Once the forms have been created, use them to enter a few more consultations.  Add yourself as a patient and then have a consultation for yourself with a date of 3/8/2010. 

Queries

You will develop two queries:  Patient Query and Patient Billing Summary.  You can use the query wizard.  If the query doesn't turn out the way you want, simply delete the query and do it again.  A good tutorial on queries in Access 2007 is in the Microsoft Access tutorials (see links).

The Patient Query will include the following:

  • Patient Last Name
  • Patient First Name
  • Treatment Date
  • Physician Name
  • Treatment Description
  • Cost
  • Notes

The Patient Billing Summary will be a summary query and will include the following for each patient who has had a consultation:

  • Patient Last Name
  • Patient First Name
  • Total Cost of all consultations
  • Number of consultations

Deliverables

For this project, you will need to turn in a copy of your database file.  This files must be attached to an email sent to me in WebCT.  It must be submitted before the deadline, 11:59 a.m. on Friday, March 26th.
